The following deficiency was found during the on-site compliance inspection and investigation of complaint 00153794 conducted on December 22, 2025:

Based on documentation review and interview, the manager failed to ensure that a disaster plan included a plan for obtaining food and water for individuals present in the assisted living facility. The deficient practice posed a risk as there was no plan to ensure the health and safety of residents in an emergency. Findings include: 1. A review of the facility’s documentation/policies and procedures revealed a disaster plan for the facility, however, the plan did not include a plan for obtaining food and water for individuals present in the assisted living facility. The plan revealed a relocation address for another assisted living facility, however, the plan did not include a plan for obtaining food and water. 2. In an exit interview, the findings were discussed with E1 and E2, and no additional information was provided.
