The facility failed to ensure that personal care aides were not assigned dietary duties such as setting tables, pouring beverages, plating food, serving meals, and cleaning dining areas. Observations showed aides performing these prohibited tasks, including setting silverware, pouring juice, and clearing dishes from the dining room.

Specifically, one resident had a chest X-ray instead of the required second step of the skin test, and another resident had a completed first step but lacked documentation of the required second step.
