3-year lookback · Medicare 2026
Families have filed complaints about this facility resulting in serious findings of actual harm, including failures to follow treatment orders and pressure ulcer care. The facility has persistent problems with medication management, infection control, and fire safety systems that recur across multiple surveys. While issues appear to be corrected after identification, the pattern of repeated deficiencies in core care areas suggests ongoing quality concerns families should investigate thoroughly.
