Key Review Excerpts

The rehab staff worked with my dad giving him the encouragement he needed to get up and walking again. The nurses and aides gave great care to him.

Rehab patient's family · 2022☆☆☆☆

The food is consistently cold and unappetizing. You are supposed to be able to get something else for your meal when you don't like what is served but the kitchen staff cuts out as soon as they send the food out to the rooms.

Long-term resident's family · 2022☆☆☆☆

She calls for someone to bring her to the bathroom or put her on her side bed Potty and it can be an hour before someone shows up. I actually had to call the nurses station to get someone to go to her room.

This facility shows concerning patterns with 61 deficiencies across 14 surveys, including many complaints filed by families. Recurring issues involve abuse prevention and reporting, medication management, and accident prevention/safety supervision. While all deficiencies show correction dates, the repeated problems in critical safety areas—particularly multiple instances of inadequate abuse reporting procedures and accident hazard prevention—suggest ongoing systemic challenges that warrant careful scrutiny during any visit.
