However, the recurring reports of neglect and slow response times are significant red flags. If you choose this facility, ensure you have a family member present frequently to advocate for your loved one's basic hygiene and medical needs.

St. Elizabeth Rehabilitation & Nursing Center receives high praise for its physical and occupational therapy departments, with many patients reporting successful recovery and improved mobility. However, the facility faces serious, recurring allegations of neglect, poor communication, and understaffing, particularly regarding basic hygiene and timely response to patient needs. Questions for Your Tour

  • 1I noticed the facility is very responsive to online feedback; how does that same level of communication translate to addressing family concerns about daily care needs?
  • 2Given the importance of timely assistance, what is your current protocol for monitoring and responding to call buttons to ensure residents aren't waiting too long for help?
  • 3Since physical and occupational therapy are noted as strong points here, how do you integrate those services into the daily routine for residents who need extra support with mobility or hygiene?
  • 4I see that medication management is a priority; could you walk me through your process for ensuring accuracy and consistency in administering medications?
  • 5How do you manage staffing consistency, particularly regarding the use of temporary personnel, to ensure residents are familiar with the caregivers assisting them with their daily sanitary needs?
  • 6What steps are you taking to improve the dining experience and ensure that meals are served at the appropriate temperature and meet the nutritional preferences of the residents?

Key Review Excerpts

The therapy has been helpful in strengthening my legs, arms and the rest of my body. The nursing staff has been great. The nursing staff is helpful, compassionate and knowledgeable.

Rehab patient · 2026★★★★★

If I could give this place a ZERO I would. My dad was there for rehab and we had to move him because be was sick & they did nothing to help him. Several times he was left sitting in his feces for over an hour calling us crying.

Long-term resident's family · 2025☆☆☆☆

I was told my mother didn’t have an infection after she had already been given 5–7 days of antibiotics, without any retesting. More than once, I was told she couldn’t have an infection because the antibiotic she was on was “very strong”.
