Based on a review of documentation and staff interviews, it was determined that the facility failed to provide supervision of the resident?s schedule, care and activities, including attention to specialized needs, such as prevention of falls and wandering from the premises. Evidence: 1) A self-report was received by the facility (Staff #1) on 08/16/2025 that resident #1 eloped the facility on 8/16/2025 at 1:40pm. Resident #1 was retrieved within 1 minute and brought back into the facility. 2) Staff #1 confirmed that the alarm was functioning properly when resident #1 exited out of the staff entry/back exit door. Due to this, staff were able to quickly retrieve resident #1 and bring resident back into the building. Upon reentering the building, Resident #1 was evaluated and was not injured during the incident.
