Specifically, the facility did not schedule a required cardiology follow-up appointment for a resident with heart failure, as the Supervisor in Charge missed the instruction in the discharge paperwork.

The facility failed to ensure that 3 of 3 sampled resident bathrooms had paper towels or hand towels available for resident use. Interviews with residents and observations of the bathrooms confirmed the absence of these supplies, despite paper towels being available in storage.

The facility failed to ensure health care referral and follow-up for 1 of 2 sampled residents. Specifically, there was a failure regarding a missed dermatology appointment and a referral for physical therapy for Resident #3.

The facility failed to ensure that two of two sampled resident bathrooms had paper towels or hand towels available for resident use. Observations and resident interviews confirmed that bathrooms lacked necessary drying materials, with some residents relying on personal towels from their rooms.
