The facility failed to administer medications as ordered by a licensed prescribing practitioner for one resident. Specifically, a physician's order for artificial tears was not faxed to the pharmacy, resulting in the medication not being available for administration and no entry appearing in the electronic Medication Administration Record (eMAR).

The facility failed to ensure that medications were administered as prescribed for 2 of 3 sampled residents. Specifically, for Resident #1, the facility failed to follow new sliding scale insulin orders for Humalog and Levemir provided by an endocrinologist, instead continuing to use an outdated scale. This resulted in the resident receiving incorrect insulin doses based on blood glucose readings.

The facility failed to ensure that medications, including Humalog, Levemir, and Lisinopril, were administered according to prescribed orders for 2 of 3 sampled residents. Specifically, for Resident #1, there was no documentation of a sliding scale for a new insulin order dated 12/11/14.
