The facility failed to ensure medications prepared in advance were properly identified and protected from contamination for three residents. Specifically, the Supervisor-In-Charge was observed pushing medications one by one from bubble packs into clear plastic medicine cups rather than using sealed or appropriately labeled containers.

The facility failed to ensure medications prepared in advance were properly identified and protected from contamination. Specifically, the Supervisor-In-Charge prepared medications for three residents in unlabeled, stacked plastic cups that were not protected from contamination or labeled with resident names.

The facility failed to administer metoprolol ER as ordered for one resident. Due to a pharmacy misinterpretation of a handwritten order, the resident was administered the medication twice daily instead of the prescribed once daily dose from July 24, 2017, through October 11, 2017.
