Key Review Excerpts

My grandmother (86years old)was transferred to this facility in early March and spent about 3 weeks here to recover from a spinal disc surgery and lower back fracture. The quality of care she received was excellent!

Rehab patient's family · 2022★★★★★

My mother spent 4 weeks at Schuyler Ridge Rehabilitation recently after taking a bad fall. They have a great system where you get feedback on your loved ones progress from all members of the team (nursing, physical therapy, occupational therapy, dietician and case manager).

Rehab patient's family · 2021★★★★★

This faculty has made my mother routinely wait to go to the bathroom for sometimes 45 minutes. It has impacted her health so much she got a UTI from it. It took them 3 days to get a sample and culture to even begin a antibiotic.

11 complaint-triggered

Families have filed multiple complaints triggering investigations, with recurring issues around protection from abuse and neglect appearing across surveys from 2023 to 2025. The facility shows persistent problems in three main areas: fire safety and emergency preparedness, care planning and assessment, and resident protection protocols. While all deficiencies show correction dates, the pattern of repeated violations in critical safety areas suggests ongoing operational challenges that families should discuss during visits.
