Questions for Your Tour

  • 1I noticed your leadership team is very active in responding to online feedback; how do you use that family input to improve daily care and communication?
  • 2Given the current staffing levels, what is your specific protocol for ensuring call lights are answered promptly and residents' personal hygiene needs are met throughout the day?
  • 3Could you walk me through your process for updating families on a resident's medical status, especially when there are changes in health or medication management?
  • 4What specific steps are you taking to address the recent CMS health inspection findings to ensure a higher standard of care for your residents?
  • 5Since the facility is large with 200 residents, how do you ensure that each individual receives consistent attention and that their specific care plan is being followed?
  • 6What does a typical day look like for a resident here, and how do you ensure they are engaged in meaningful activities rather than just staying in their rooms?

Key Review Excerpts

The transition between SGHS and PruittHealth that took place in September 2024 was a little rough and for some time afterwards there weren't enough CNAs during the late shifts or weekends to meet the residents' needs. Recently, it has improved and when I visit, there has appeared to be enough staff on the floor.

Long-term resident's family · 2025★★★★

The staff are consistently inattentive; calls for assistance often go unanswered for up to an hour or more, even for urgent needs. Personal hygiene and room cleanliness are largely neglected, resulting in unsanitary conditions.

Family member · 2025☆☆☆☆

Porscha is absolutely wonderful to work with. She’s so knowledgeable, helpful, and always goes above and beyond for her residents. It’s truly a pleasure collaborating with her on discharges – she makes the process smooth and efficient every time.
