The facility underwent an unannounced case management visit to follow up on eviction notices and an unlawful detainer. The Licensing Program Analyst met with the Licensee and Administrator, and based on the information gathered, no citations were issued on this date.

This report details a complaint investigation initiated on October 23, 2025, regarding unsanitary conditions. Following an unannounced visit and review of records and interviews, the allegation was deemed UNSUBSTANTIATED. The investigator observed that common areas and resident rooms were clean and sanitary.

The facility was inspected for its required 1 Year Annual evaluation and was generally found to be clean and sanitary. However, one significant deficiency was cited regarding the unsecured access to self-managed medications, which presents an immediate health and safety risk. The facility was advised to regulate hot water temperatures and secure medications in shared units.
