What this means for your family

This facility offers strong physical and occupational therapy, making it a viable option for short-term rehabilitation. However, families should be aware of persistent reports regarding understaffing and slow response times to call lights; we recommend visiting during off-hours to observe staffing levels firsthand.

Key Review Excerpts

The only reason I have it two stars instead of one is because it is a newer facility, fairly clean, and the physical therapists are there most days. Otherwise, it is grossly understaffed, nearly impossible to get anyone to answer the phone or call you back.

Long-term resident's family · 2025★★☆☆☆

I’m very grateful for the care my mom received at Salem Transitional Care. She stayed there for about a month, and the RNs and LPNs were consistently patient, kind, and attentive. While the facility did seem a bit understaffed at times, the quality of care never wavered.

Long-term resident's family · 2026★★★★

The doctors in the day time are great, but the night crew is terrifying. They are poorly trained, they allow their mood and emotions to dictate their treatment of passionates.
