What this means for your family

While the facility has a strong reputation for short-term physical therapy, the recent, severe reports of medical neglect are deeply concerning. If you choose this facility, maintain a very active presence, monitor your loved one's health markers daily, and do not hesitate to escalate concerns to external authorities if you feel your requests for information are being ignored.

Questions for Your Tour

  • 1I noticed your team is very active in responding to feedback online; how do you use that family input to improve the daily experience for residents?
  • 2Given the importance of consistent wound care and hydration, what is your specific protocol for monitoring these needs and keeping families updated on any changes in a resident's physical health?
  • 3With your focus on short-term rehabilitation, how do you ensure that long-term residents receive the same level of consistent attention and hygiene support throughout the day?
  • 4I see your staffing rating is a 3/5; could you walk me through how you manage shift handoffs to ensure that communication about medication and care needs doesn't fall through the cracks?
  • 5What does a typical social calendar look like for residents, and how do you encourage participation for those who might need a little extra help getting to activities?
  • 6How do you handle urgent medical situations, and what is your process for notifying a family member immediately if a resident's health status shifts?

My mother was there maybe 3 wks and got a severe bed sore that they refused to let us see . Her mouth wasn't taking care over . On the 3rd week of her being there she wasn't acting right and wasn't responding the greatest. Had them send her out only to find out from the emt that her sugar was above 600 and several diehited.
