Key Review Excerpts

My stepfather ended up with a bedsore the size of platter, clogged feeding tube, and mold growing in his oral care products that were not being used despite several complaints from my Mom and stepbrother.

Family member · 2018☆☆☆☆

Called twice( after being put on hold) before I actually got to speak to a very rude nurse. Then I explain to her I was worried about a family member that needed to be checked on and she says they have a phone in their room.

Family member · 2021☆☆☆☆

Tried to call a family member there, waited 10 min on hold, called back 3 times asked to speak to my family member was told they only have 1 phone and couldn't locate it so I never got to speak to said family member...what a mess

This facility shows recurring safety concerns with five deficiencies for accident hazards and supervision across multiple surveys, plus persistent issues with fire safety systems and food quality. Families have filed complaints leading to citations for communication failures, accident hazards, and respiratory care problems. While all deficiencies appear corrected, the pattern of repeated safety violations in accident prevention suggests ongoing supervision challenges that warrant careful evaluation during visits.
