Key Review Excerpts

My uncle sat in his feces for 4 hours before being changed. He has a terrible rash because of it, and has also developed an awful bedsore which the nurses failed to mention to the family.

Long-term resident's family · 2022☆☆☆☆

Multiple times they forgot to give her food trays and when they did this is what her trays looked like, not always but mostly inedible food. After a week we had them transport her back to the hospital where she was malnourished.

Rehab patient's family · 2026☆☆☆☆

I missed meals there, missed meds there, missed rehab times there. It took me a while but I AMA (Against Medical Advise) out of there and when back out to the VA ER.

Saint Joseph Transitional Rehabilitation Center shows concerning patterns with 64 deficiencies across three years, including families filing complaints about daily care assistance.
