The facility failed to ensure the building and structure complied with Chapter 18, New Health Care Occupancies, of the Life Safety Code, NFPA 101.
The facility failed to provide record drawings of the fire detection and alarm system as installed as required.
The facility failed to provide and maintain heating and cooling systems in compliance with licensing standards for assisted living facilities.
The facility failed to provide copies of the fire sprinkler system approval, and/or a copy of the hydraulic calculations, alarm configuration, above ground and underground Contractor's Material and Test Certificate proper manufacturing instructions.
The facility failed to provide a kitchen staff restroom that was in compliance with licensing standards for assisted living facilities.
The facility failed to ensure an emergency generator was in compliance with licensing standards for assisted living facilities.
The facility failed to provide kitchen janitorial facilities that were in compliance with licensing standards for assisted living facilities.
The facility failed to ensure waste, trash, and garbage disposal was disposed of in accordance with state and local requirements, and/or excessive accumulations were not taken care of, and/or the facility failed to comply with requirements for Disposal of Special Waste.
The facility failed to ensure interior walls and/or ceilings were constructed with a material having at least a 20-minute fire rating, or at least 3/8 gypsum board.
The facility failed to provide operable windows or powered exhaust that is required for all odor-producing rooms.
The facility failed to provide the required sprinkler system to meet NFPA 13.
The facility failed to provide adequate janitor closets.
The facility failed to secure written approval of the local authorities having jurisdiction, including the fire marshal, health department, and building inspector.
