FACILITY EVALUATION REPORT

The facility underwent a required 1-year annual inspection and was generally observed to be in good order, with adequate supplies and operational safety equipment. However, one Type B deficiency was cited regarding the condition of a backyard gate, which requires repair to ensure resident safety. All other areas inspected, including linens, food supplies, and documentation, were found to be compliant.

The inspection was an unannounced annual required visit. While the facility was generally found to be clean and well-stocked, two deficiencies were cited. These relate to staff training compliance and the required review of resident care plans.

The unannounced Annual Required inspection found the facility generally clean with appropriate safety measures observed, such as functioning smoke and CO detectors. However, one deficiency was cited regarding staff training records, indicating that staff require updated annual RCFE training. The facility was given a Plan of Correction due by 07/31/2023 to address this training gap.

The inspection revealed several deficiencies, including an immediate safety hazard from unsecured tools, evidence of poor sanitation (spider webs), and staff training gaps. The facility was otherwise observed to be clean, and the administrator was cooperative during the unannounced visit.

The facility underwent an unannounced Case Management visit to follow up on a resident's (R1) AWOL status. The licensee cooperated fully with the investigation, providing documentation and technical assistance to the licensing authority. No deficiencies were cited during this visit.

The inspection was an unannounced Required 1 year visit. While the facility was generally found to be clean and safe, several deficiencies were noted regarding safety equipment, unsecured items, and medication storage. These issues required immediate corrective action by staff during the visit.
