Questions for Your Tour

  • 1Given the current staffing levels, what is the average response time when a resident uses their call light for assistance with hygiene or mobility?
  • 2I understand that medical oversight is a priority; could you walk me through your specific protocols for identifying and escalating a resident's sudden change in health status to a physician?
  • 3What steps is the facility taking to improve the quality and variety of the dining experience, as we've heard feedback that the food service could be more consistent?
  • 4How do you ensure that staff maintain a patient and compassionate approach when assisting residents with daily needs, especially during busier times of the day?
  • 5With the facility's focus on physical and occupational therapy, how do you integrate those recovery goals into the resident's daily routine outside of scheduled sessions?
  • 6What is your standard process for communicating updates about a resident’s health or medication changes to family members to ensure we are always kept in the loop?

2 complaint-triggered

This facility has faced recurring safety and care concerns, with families filing complaint reports that led to findings of abuse/neglect and inadequate daily care assistance. The most frequent problem areas involve fire safety systems, followed by resident protection from abuse and food safety. While all deficiencies show correction dates, the pattern of repeated fire safety violations across multiple surveys and serious complaint-triggered issues regarding resident protection suggest ongoing challenges with maintaining consistent safety standards.
