Failed to ensure a valid background check was submitted every two years for 2 of 2 staff (Staff E and F).
The facility failed to ensure tuberculosis screening was completed within three days of hire for 1 of 3 staff (Staff D). Staff D began working on 09/26/2024, but the TB test was not completed until 11/14/2024, 49 days later.
Failed to complete mandatory orientation and safety training before staff (Staff B and C) interacted with residents, and failed to maintain current CPR/first-aid certification for staff (Staff B and F).
Failed to ensure a background check was submitted within one day of start date and failed to obtain three reference checks for 1 of 3 staff (Staff D).
Failed to ensure a national fingerprint background check was completed within 120 days for 1 of 3 staff (Staff C).
