Specifically, the auditory alert device was found to be intermittently turned off by staff, which presents an immediate and direct risk to the safety of residents at risk for elopement. Corrective action requires immediate and continuous activation of this safety device.
The facility must ensure an auditory device or other staff alert feature is active at all times on exterior doors and perimeter fence gates accessible to residents at risk for elopement. Intermittent use of the device poses an immediate health and safety risk.
Staff were observed intermittently turning the auditory alert device on and off at the front entry door. The device must remain activated continuously to monitor exits.

Furthermore, a critical deficiency was noted regarding Staff #1 administering medication without completing the required six hours of hands-on training, posing a potential health risk.
For facilities licensed to provide care for 15 or fewer persons, the employee shall complete 10 hours of initial training. This training shall consist of 6 hours of hands-on shadowing ... and 4 hours of other training or instruction,... first two weeks of employment.
Staff #1 have not completed six hours of medication training prior to administering medication to Resident #1 which posed a potential health risk to resident in care.
