May 5, 2026Inspection
A follow-up inspection on 05/21/2026 found no deficiencies, indicating the identified WAC 388-78A-2660 and RCW 70.129.140 violations were corrected.
The facility failed to provide care with dignity for 1 of 4 sampled residents (Resident 2), resulting in the resident being spoken to in a condescending manner by a staff member.

The inspection on 12/17/2025 resulted in a 'Disapproved' status due to deficiencies. The follow-up inspection on 01/13/2026 resulted in an 'Approved' status after the Forward Flow report from IEFP was received.
The south door did not close and latch when tested. Corrected at inspection.
Forward flow testing of the backflow preventers shall be required per NFPA 25 13.7.2.

A separate cover letter indicates that WAC 388-78A-2090-6-e and WAC 388-78A-2090-1 were corrected as of the 12/02/2024 follow-up inspection.
The facility failed to complete medical device safety assessments for 2 residents (Residents 2 and 5) who were using bed rails.

The facility failed to accommodate a resident's request for help with insulin dosage verification, failed to attempt to preserve placement, and failed to include required advocacy information and location of discharge on a written discharge notice.
