What this means for your family

While the facility is frequently praised for its clean environment and effective physical therapy, the recurring reports of nursing negligence and poor dementia care are significant red flags. We strongly recommend that families tour the specific unit where their loved one will stay and ask direct questions about how the facility handles pain management and patient responsiveness during off-hours.

Key Review Excerpts

My husband recently spent 2 weeks at Quaboag for rehab. I cannot say enough positive about his stay- Taghi in PT, Patti in OT, RN- Erica and Beth & Brandi as PCAs(even Jada who was in training) all went above and beyond to help him

Long-term resident's family · 2023★★★★★

PLEASE DO NOT SEND YOUR LOVED ONE TO THIS FACILITY! ABSOLUTELY DISGUSTING ON THE CARE OF OUR 82 YR. OLD FATHER! NOT ONLY ARE THEY INCOMPETENT OF TAKING CARE OF SOMEONE WITH A SMALL BOUT OF DEMENTIA, THEY ARE UNPROFESSIONAL, NEVER TAKE FAULT OF THEIR LACK OF CARE!

Family member · 2024☆☆☆☆

Every one of them was friendly to Nan and I. All were knowledgeable, timely, accommodating and frequently provided tips to help me in my future care giving.

Clean record — no deficiencies

This facility has had families file complaint reports, leading to findings around resident dignity and care quality standards. The most recurring issues involve medication management (including unnecessary drugs and dosing protocols) and personal care for incontinence, with the latter appearing in multiple surveys. While all deficiencies show correction dates, the pattern of medication oversight problems and repeat personal care issues, combined with complaint-triggered violations, warrants careful evaluation during visits.
