Key Review Excerpts

The nurses would take it upon themselves to not give him the pain meds that was prescribed to him. They left him in a recliner all day and night and he ended up with a open sore on his buttocks.

Long-term resident's family · 2024☆☆☆☆

The facility itself is spotlessly clean and well-maintained, creating a bright and comfortable environment. From the front desk staff to the care team I observed interacting with residents, everyone was incredibly courteous and genuinely attentive.

Visitor · 2026★★★★★

It took over an hour for a nurse to come . No one is in the same page! They have no clue as to what they are doing! They don’t follow doctors orders! Gave him full sized pills and didn’t crush them cause him to aspirate.

Providence Care Center has significant recurring issues with families filing multiple complaints about abuse and neglect prevention, with recent citations for improper restraint use and failure to report suspected abuse. The facility repeatedly struggles with basic quality of care including pressure ulcer prevention, incontinence care, and medication safety, along with persistent safety hazards and infection control problems. While issues appear corrected after citations, the pattern of complaints and recurring deficiencies in fundamental care areas raises concerns.
