As a family member whose mother had the misfortune to be a patient in the skilled nursing unit at Presbyterian Village and died there just prior to the state survey being conducted in which numerous violations were discovered that resulted in a one star rating being assigned to the facility by the Centers for Medicare Services (CMS), I found the report disturbing.

Independent living may be fine, but assisted living and skilled nursing were horrible at best. My mother died there. Not long before, I was told by another resident’s family member that they found my mom on the floor under her bed screaming. We were never notified.
