The facility failed to maintain a matching therapeutic diet menu for a resident prescribed a mechanical soft with chopped meats diet. During the survey, staff were unable to use a specific menu for guidance, and the resident was served from the regular diet menu. An interview with a personal care aide confirmed they were unaware of the requirement to use diet extensions for therapeutic diets.
