Pleasant Valley Adult Care Home, LLC

Based on observation, record review, and interview, the manager failed to ensure at least the manager or a caregiver was present at an assisted living home when a resident was present in the assisted living home. The deficient practice posed a risk as no qualified employee was present to meet a resident's needs for an undetermined amount of time. Findings include: 1. The Compliance Officer observed E3 alone and working on the premises at 8:30AM. 2. In an interview, E3 reported the facility had five residents. 3. The Compliance Officer observed E2 arrive on the premises at approximately 8:37AM. 4. The Compliance Officer observed five residents during a tour of the facility. 5. A review of E3's personnel record revealed E3 was hired as an assistant caregiver. 6. A review of E2's personnel record revealed E2 was hired as a caregiver. 7. In a joint interview, E1 and E2 reported E3 was an assistant caregiver. E1 and E2 reported E3 did not have documentation of completion of caregiver training. 8. In an interview, E1 reported E2 had left AL10750 to go to a house down the street. E1 acknowledged there was no manager or caregiver present at the assisted living home when residents were on the premises.
