What this means for your family

While some families report positive rehabilitation outcomes and engaging activities, the facility has a concerning history of cleanliness issues and reports of medical neglect. We strongly recommend conducting an unannounced visit to inspect the cleanliness of resident rooms and asking specifically how the facility manages communication during medical emergencies.

Key Review Excerpts

He came there walking only with a cane, he left in a wheelchair, never to walk again. He also got a bedsore on his heel, and Therapy rigged up these long plastic velcro “things” for him to wear that were too long, and said they couldn’t make them shorter!

Long-term resident's family · 2024☆☆☆☆

Mother sent for rehab. Fell three times. Dehydration, UTIs, falls, C diff, ect. People aren't priority. This administration should be ashamed!

Rehab patient's family · 2024☆☆☆☆

The staff is so nice and treats me just like family. This is the cleanest and most well taken care of facility I have been in. There are always fantastic activities and games for us to enjoy throughout the week including arts and crafts and bingo 2 times a week.

This facility shows concerning patterns with 108 deficiencies and frequent family complaints triggering inspections. Recurring problems include accident prevention and safety supervision, treatment quality, and protecting residents from abuse and neglect - with these same issues appearing repeatedly across multiple surveys. While all deficiencies show correction dates, the persistent nature of safety and care quality problems, combined with the high volume of family-initiated complaints, suggests ongoing operational challenges that warrant careful evaluation during a visit.
