Patient wonders throughout the facility alone with out supervision staff do not know where is the patient, no bedding linen available , found patient smelling like urine, no dippers exchange, working whithout shoes patient personal items missing.

I called ahead of time and spoke with the SW. I was told that there would be activities daily for him to participate in and that he would have his meals in the dining room. I later learned that he only left his room to visit another resident.
