Questions for Your Tour

  • 1I noticed the facility has a strong reputation for being welcoming and responsive; how do you ensure that same level of engagement translates into daily activities for residents who are physically mobile?
  • 2Since the facility has a 4-star staffing rating, how do you utilize that staff to help encourage residents to get out of bed and participate in social activities throughout the day?
  • 3Given the recent health inspection reports, what specific steps is the facility taking to improve those areas and ensure the highest quality of care for your 55 residents?
  • 4How does your team handle medical emergencies or sudden changes in a resident's health status, especially during overnight hours?
  • 5With 4 state violations noted in the past year, could you walk me through how you address and resolve these types of concerns to maintain the clean and well-maintained environment mentioned by families?
  • 6What kind of daily routine or movement program do you have in place to ensure residents remain active and engaged, rather than spending the majority of their time in their rooms?

Key Review Excerpts

The only thing that bothered me was, the poorest patients, 2 to a room, were all laying in the bed with Venetian Blinds closed at around 10 in the morning! I know some cannot get up they are bedridden… But this then can….should be… I believe a chair, a small cheap one should be beside every bed!

Families filed complaints leading to serious findings including abuse/neglect protection failures and accident prevention issues in 2025. The facility shows recurring problems with fire safety systems and resident rights across multiple surveys from 2020 to 2024. While all deficiencies have correction dates, the pattern of ongoing fire safety violations and recent complaint-triggered findings about resident protection raise concerns about consistent care quality.
