Key Review Excerpts
“The staff at this facility are the best around. Top tier. Miss Kalyn does an amazing job getting activities scheduled for the residents, along with community outreach. Having suffered from a life threatening TBI, and survived, this is where I would have received my care if I had to do that again.”
“The door to the smoking area shut on her ankle. Now we are working on 12 weeks in a cast. No physical therapy with a broke ankle. Her next appointment is September 25th. This is the 4th cast. Her bones are not healing correctly because she has been putting weight on her casted leg. The aides have been making her to use the toilet. The bathrooms are not handicapped compliant.”
“Love being able to bring your FUR BABY. Pecan Creek Healthcare understands the bonds between our pets and ourselves.”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
This facility shows recurring safety concerns across three surveys, with fire safety, emergency exit access, and infection control being the primary problem areas. The same deficiencies for blocked corridors and faulty smoke barriers have persisted across all three inspections from 2023 to 2025, indicating ongoing safety risks. While the facility has correction plans for most recent issues, the pattern of repeated violations suggests implementation challenges that families should carefully consider.
