A relicensure survey was completed on 12/15/25. Deficiencies were cited. Based on record review and interview, the residence failed to have emergency policies addressing all required elements, affecting seven current residents. Findings include: The residence' s Emergency Manual, dated 3/11/22, failed to contain the required element: Storage and preservation of medications. On 12/15/25 at 12:15 p.m., the administrator stated the residence had a plan on how to store and preserve medications in case of an emergency; however, she was unaware that the storage and preservation of medications was a required element of the emergency plan. Based on record review and interview, the residence failed to have policies and procedures to ensure the continuation of care to all residents for 72 hours following any emergency, affecting seven current residents.Findings include: On 12/15/25 at 9:02 a.m., the 72-hour continuation of care policy and procedures was requested; however, it was not provided. On 12/15/25 at 12:15 p.m., the administrator stated she was aware the residence had to have a 72-hour continuation of care plan; however, she was unaware that a policy was required.
