A relicensure survey was completed on 12/15/25. Deficiencies were cited. Based on record review and interview, the residence failed to have emergency policies addressing all required elements, affecting seven current residents. Findings include: The residence' s Emergency Manual, dated 3/11/22, failed to contain the required element: Storage and preservation of medications. On 12/15/25 at 12:15 p.m., the administrator stated the residence had a plan on how to store and preserve medications in case of an emergency; however, she was unaware that the storage and preservation of medications was a required element of the emergency plan. A licensure revisit was completed on 9/18/23 for all previous deficiencies cited on 1/17/23. The residence is in compliance with all regulations surveyed.
