The facility failed to maintain compliance with the Office of State Fire Marshal (OSFM) inspections, specifically regarding a missing sprinkler escutcheon ring in the dining room and loaded (dirty/dusty) sprinkler heads in the laundry room and near room number one.

The facility failed to report an allegation of sexual abuse against a resident to the department's Complaint Resolution Unit as required.

The facility failed to immediately report an allegation of financial exploitation involving a resident's stolen property (watch, phone, laptop) to the department abuse/neglect hotline.

Facility lacked a system to prevent cross-contamination of clean and dirty laundry; used open baskets in hallways.

The front porch had dark liquid debris on the walls.

Facility failed to complete an annual assessment for Resident 4 who was admitted in 2017.

Numerous cigarette butts were found on the ground and in the bushes around the front entrance, porch, and smoking area.

Facility failed to ensure privacy for Resident 4 during an injection in the hallway with buttocks exposed in view of others.

A 3-foot by 5-foot area on the exterior of the back of the building was missing exterior siding and had soft, wet, wood exposed.

Bathroom vanities need to be replaced and the cement in the courtyard needs to be repoured as stated by staff.

Numerous environmental hazards including holes in flooring, dirt/debris, inoperable vents, rusted fixtures, and exterior structural issues.

The front entrance door, doorstep, and porch had chipped paint.
