Families have filed multiple reports triggering nearly 30% of the facility's 157 deficiencies, raising concerns about resident protection from abuse and neglect, medication management, and care planning. The most recent survey in September 2024 identified 45 new violations spanning safety systems, emergency preparedness, and basic resident rights. While deficiencies appear corrected after identification, the recurring pattern of complaint-driven investigations suggests ongoing quality concerns that families should carefully consider.
