FACILITY EVALUATION REPORT

The inspection was a Case Management - Annual Continuation visit conducted on 11/14/2025. One Type B deficiency was cited regarding staff training records, specifically the lack of documentation for 20 hours of required annual training in several staff files. The facility was advised to submit an updated LIC500 by 12/14/2025.

The facility underwent a required 1-year annual inspection. The inspection noted several positive aspects, including appropriate furnishings, locked storage, and adequate food/emergency supplies. One deficiency was cited regarding the condition of a service room counter in the Willow Creek Assisted Living unit.

The facility underwent a Case Management visit concerning a report of suspected financial abuse. The visit involved reviewing documentation and confirming that the resident in question has the capacity to make their own financial decisions. No citations were issued during this inspection.

COMPLAINT INVESTIGATION REPORT

The inspection identified multiple deficiencies across several areas, including immediate health risks related to water temperature, and several areas of non-compliance regarding resident care planning, staff training, and record keeping. The facility must address these citations by the specified Plan of Correction due dates to ensure resident safety and regulatory adherence.
