The following deficiency was found during the on-site compliance inspection and investigation of complaint AZ00218420 conducted on November 19, 2024:
Based on record review and interview, the manager failed to ensure when a resident had an incident that resulted in the resident needing medical services, a caregiver immediately notified the resident's primary care provider, for one of two residents reviewed who had an incident that resulted in the resident needing medical services. The deficient practice posed a health and safety risk. Findings include: 1. A review of R1's medical record revealed an incident report dated October 2024. This report revealed R1 was aggressive when caregivers tried to get R1 up and ready for a shower. Bruising to the left hand was noted. However, there was no documentation of a call being made to R1's emergency contact/POA. 2. In an interview, E1 acknowledged R1's medical record did not include documentation that showed a caregiver immediately notified the resident's emergency contact/POA.
The following deficiency was found during the on-site compliance inspection conducted on July 17, 2023:
Based on record review and interview, the manager failed to ensure a medication was administered in compliance with a medication order, for one of two residents reviewed. The deficient practice posed a risk if the resident experienced a change in condition due to improper administration of medication. Findings include: 1. A review of R2's medical record revealed a current written service plan dated in May 2023. This service plan indicated R2 received medication administration. 2. A review of R2's medical record revealed a signed medication order dated June 28, 2023. This medication order stated "Amlodipine 5 m take one tablet once a day. Take blood pressure daily every afternoon." 3. A review of R2's medical record revealed a July 2023 medication administration record. However, there was no documentation to demonstrate R2's blood pressure was taken daily every afternoon. 4. In an interview, E4 acknowledged the blood pressure reading was not taken in the afternoon per the medication order instructions.
