The facility failed to ensure medication labels were correct for 1 of 3 sampled residents regarding the transfer of medications. For Resident #1, a bottle of Warfarin 1.25mg was found with an expired expiration date of 03/31/23 and an obscured fill date.

Specifically, three residents were observed being served water and orange juice in plastic disposable cups instead of non-disposable containers. The Administrator stated that disposable cups were used because they were easier and expressed unawareness of the requirement for non-disposable settings.

The facility failed to ensure that 3 out of 3 sampled residents were tested for Tuberculosis (TB) disease in compliance with required guidelines. Specifically, there were no TB skin test results available for review for Resident #1 and Resident #2 upon their admission to the facility.
