Key Review Excerpts

This place I was sent to has an exceptional staff who I see day in and day out go above and beyond what is required of them to keep there clients happy and well fed and comfortable on an everyday basis

Rehab patient · 2026★★★★★

The staff have been known for not reporting incidents about patients and the social worker doesn’t follow up on concerns. The staff can’t answer my basic questions of concern, I’ve witnessed them feed my mother expired food and their excuse was they didn’t know.

Long-term resident's family · 2024☆☆☆☆

The DON is the best nurse that has ever existed! Always so kind and friendly towards every patient that she comes in contact with as well as all visitors.

Palms Care Center has a concerning pattern with 62 deficiencies across multiple surveys, including families filing complaints about accident prevention and safety hazards. The facility shows persistent issues with care planning and resident assessment, infection control, and fire safety systems appearing repeatedly across different time periods.
