What this means for your family

Otterbein Lebanon offers an exceptional lifestyle for independent seniors, but the quality of care in the nursing and memory care units is inconsistent and concerning. If you are considering this facility for a loved one requiring high-level assistance, we strongly recommend conducting unannounced visits and specifically observing the cleanliness and staff-to-resident interaction in the memory care or nursing halls.

Key Review Excerpts

My grandfather was in Matthew Hall a dementia unit. The place was dirty and dark and they do not change residents for hoursss. My grandfather was there for about 3 months had multiple occasions we came to visit and found him with pee up to his chest because of how long he had dirty underpants.

Memory care family member · 2025☆☆☆☆

The care that my dad and mom received at Otterbein Transitional care unit was second to none. The minute we walked in the door we were treated with such kindness and attention to care.

Rehab patient's family · 2024★★★★★

I am an emergency operator, facility refused to answer phone call multiple times causing about a 15 minute delay to our entry. That could be life or death of your loved one.

5 complaint-triggered

Recent complaints filed by families highlight concerns about medication management, care planning, and infection control, with deficiencies recurring across multiple surveys from 2019-2025. Fire safety issues have persisted throughout this period, appearing in nearly every inspection. While all violations show correction dates, the pattern of repeated problems in medication safety and fire system maintenance suggests ongoing operational challenges that warrant careful evaluation during a visit.
