What this means for your family

While the facility is visually appealing and offers strong physical therapy, the recurring reports of nursing staff inattentiveness and poor communication are significant red flags. We strongly advise you to conduct unannounced visits during weekends or evenings to observe staffing levels and cleanliness firsthand before making a decision.

Key Review Excerpts

The nurses are not attentive. We found mom unresponsive and the aid said she was just sleeping. They finally called and ambulance and the next day she died.

Long-term resident's family · 2024☆☆☆☆

They are gentle with residents, good food, open spaces, nice rooms with Windows, ac, and tv, and have a big fish tank in lobby. The staff is very sweet and kind. Sometimes it takes a while for them to come when you hit the call button and they don't always have the activities as advertised on the calendar which is disappointing.

Family member · 2017★★★★

I would like to add my great appreciation and respect for the quality of treatment my mother has received at Old Jefferson. Admission was a smooth process with a counselor who provided a wealth of information and responded to my calls quickly.

1 complaint-triggered

This facility has recurring deficiencies across multiple care areas, with families filing four complaints that triggered inspections. The most persistent problems involve resident assessments and care planning (appearing in three separate surveys), food safety and dietary services, and daily care assistance. While all 14 deficiencies have correction dates, the pattern of repeated issues in resident assessment and dietary services suggests ongoing challenges families should discuss during visits.
