While the facility offers a beautiful environment and excellent daytime programming, families should be aware of reported inconsistencies in nighttime care. If you choose this facility, we strongly recommend frequent, unannounced visits during off-hours and maintaining open, documented communication with management regarding care plans.

Questions for Your Tour

  • 1Given the facility's beautiful setting and focus on social engagement, what specific activities are planned to keep residents active and connected throughout the week?
  • 2With the current staffing ratings, could you walk me through your nighttime protocols to ensure residents are checked on regularly and receive the support they need during those hours?
  • 3I noticed some concerns regarding medication management; could you explain your internal process for ensuring medications are administered accurately and on schedule?
  • 4How do you document and verify that daily hygiene and personal care needs are met, especially if a resident is feeling resistant to assistance on a particular day?
  • 5I appreciate that your team is active in responding to feedback online; how do you use that resident and family input to improve the quality of care in the nursing unit?
  • 6Since the facility has a smaller capacity of 23 residents, how does this intimate environment help your team provide more personalized medical oversight compared to larger centers?

Key Review Excerpts

The problem is follow-up, on problems that occur. Our mom did not get the help she needed to bathe. Second and third shift did not regularly respond when she pushed her button requesting help to the bathroom.

Long-term resident's family · 2024★★☆☆☆

The case managers at the hospital wanted to send her to a nursing home. Thank goodness for Brittany Marshall who is the Health and Wellness Director. Brittany stepped in and advocated for mom with the case managers and helped us set up a plan for getting mom back to Thrive.

Long-term resident's family · 2024★★★★★

Do not send your loved ones to the assisted living or memory care. These people do not care! Massive decline with our grandmother. Drugged up, no interaction or activities, filthy clothes, unbrushed teeth.

Clean record — no deficiencies

Families have filed complaints that triggered recent inspections revealing deficiencies in care planning, safety supervision, and infection control. The facility shows recurring problems with fire safety systems and emergency preparedness across multiple surveys from 2022-2025. While all deficiencies have reported correction dates, the pattern of repeated violations in building safety and the complaint-driven care issues warrant careful consideration during any visit.
