The inspection was a Case Management visit to follow up on an AWOL incident that occurred on 09/30/25. Two deficiencies were cited, both classified as Type A, indicating immediate risks to resident health and safety. These relate to insufficient staffing competency and the resident's ability to leave the facility unassisted.

The facility underwent a Case Management visit conducted by an LPA. The visit included a general Health and Safety check covering food supply, physical plant, and staffing. No deficiencies were observed pursuant to Title 22 rules and regulations, Health and Safety Codes.
