What this means for your family
While the physical therapy department receives praise for helping patients regain independence, the facility has documented, high-severity failures in medication management and basic resident care. We strongly advise families to look elsewhere, or if you must choose this facility, ensure you have a daily presence to monitor medication administration and hygiene needs.

19 reviews on Google“Oakridge Gardens Nursing Center presents a highly inconsistent experience, with significant concerns regarding clinical safety, staffing levels, and management professionalism. While some families report positive experiences with individual staff members and rehab services, multiple reports of medication errors, delayed call-light responses, and poor hygiene standards raise serious red flags. Families should exercise extreme caution and conduct thorough due diligence before considering this facility for a loved one.”

Key Review Excerpts
“At first I thought it was good for 93 yo Mom. But then, they gave her 17 medications that were for another patient! THESE WERE NOT HER MEDS! This woman had to have her food pureed, how did they force so many pills into her? Her body could not recover from that ,and she died a few days later!!”
“My brother is there for rehab. Everyone is so nice. From the physical therapy occupation staff nurses ,cna, laundry staff , office staff and housekeeping . They answer all our questions and keep us in the loop as to how he is doing.”
“My nana pressed the call light for these nurses to come help her to the bathroom and she ended up having an accident because it took the nurses an HOUR AND A HALF to come answer her call light.”

The most problematic areas include fire safety systems (with repeated fire alarm deficiencies), nutrition and dietary services, and infection control practices.
