This report details the findings of a complaint investigation concerning allegations of staff neglect, including resident hospitalization, leaving a resident soiled, and failure to follow care plans. After reviewing records and interviewing staff and sources, the investigation concluded that the allegations were unsubstantiated due to a lack of corroborating evidence.

The facility underwent an unannounced annual inspection. The LPA reviewed client records, personnel files, and conducted facility tours, noting that the facility was generally clean and in good repair. The report explicitly states that zero deficiencies were observed.

The facility underwent an unannounced annual inspection on August 28, 2023. While many areas, including infection control and physical plant, were found to be in order, a significant deficiency was noted regarding resident medical records. Specifically, 70% of resident records were missing current physician's reports, which constitutes an immediate health risk.

The facility was cited for failure to properly reappraise the resident following a change in condition. Specifically, the report notes that the required pre-admission appraisal was not updated to reflect the resident's changing health needs related to a wound. These deficiencies are classified as Type A, indicating an immediate health and safety risk.
