The facility failed to ensure that a resident's assessment was completed. Specifically, for one of three sampled residents, there were no care plans available for review after January 20, 2021.

The facility failed to administer medications as ordered for one resident. A prescribed course of prednisone following an emergency department visit was not entered into the resident's Medication Administration Record (MAR), resulting in a failure to document and potentially administer the medication.

The facility failed to ensure that one of three sampled residents had a resident assessment completed annually. Specifically, Resident #2 did not have any care plans available for review after 01/20/21, despite the resident's need for assistance with activities of daily living.

The facility failed to ensure that two out of three reviewed staff members who administered medications were clinically validated for competency. Personnel files for Staff B and Staff D lacked documentation of completed clinical medication administration validation.

The facility failed to assure that two out of three reviewed staff members (Staff B and D) were competency validated for Licensed Health Professional Support (LHPS) tasks. There was no documentation in the personnel files to prove these staff members had completed the required competency validation via return demonstration.

The facility failed to ensure that two out of three reviewed staff members who administered medications had completed the required clinical skills validation. Specifically, personnel files for Staff B and Staff D lacked documentation of a completed clinical medication administration validation by a registered nurse or pharmacist.
