Key Review Excerpts

Has had wet diaper more than one time and she said herself she had to wear it all night. So desperately unhappy.

Long-term resident's family · 2024☆☆☆☆

Linda (secretary) is AWESOME! I called to wish my brother Happy Birthday because I coud not travel from St. Louis. Linda went out of her way went to his room, and used her cell phone to call me for him.

Long-term resident's family · 2021★★★★★

If you care about your loved one, do not put them here. They are understaffed and neglect patients. Once I was there and instead of employees helping any patients, they were running up and down the hall playing chase while talking about a upcoming football game.

Medicare Inspection History

10deficiencies
Above state avg (6.7)
16 complaint-triggered

This facility has a concerning pattern of repeated deficiencies, with nearly half (12 of 27) triggered by family complaints. The most persistent problems involve care planning, daily care assistance, and medication management, with these same issues recurring across multiple surveys from 2022 to 2025. While the facility corrects violations when cited, the repetitive nature—especially in core care areas—suggests ongoing quality challenges that families should carefully investigate during visits.
