Based on record review and interview, the administrator failed to ensure a personnel member's skills and knowledge were verified and documented before the personnel member provided physical health services and according to policies and procedures, for two of two sampled personnel. The deficient practice posed a risk if employees were unable to meet the needs of residents. Findings include: 1. A review of E1's (hired in 2022) personnel record revealed no documentation of skills and knowledge verification. 2. A review of E2's (hired in 2024) personnel record revealed no documentation of skills and knowledge verification. 3. In an interveiw, E3 acknowledge E1's and E2's skills and knowledge were not verified and documented before the personnel member provided physical health services.

Based on record review and interview, the administrator failed to ensure a resident does not share any space, participate in any activity, verbally or physically interact with another resident that based on the other resident's develop level, social skills and personal history may present a threat to the resident's health and safety. Findings include: 1. A review of R1 medical record revealed a document titled incident report dated July 24, 2024, which reflected R1 was sitting outside during R1's one on one staff and became angry and tried to grab the staff member by the hair. R1 chased the staff member with the Ipad and hit the staff across the face. R1 was de-escalated by the facility's house manager. 2. A review of R1 medical record revealed a document titled incident report dated July 22, 2024, which reflected R1 became upset with R3 and ran to the living room to attack R3, Staff move quickly to have R1 release R3's hair. 3. A review of R1 medical record revealed a document titled incident report dated July 23, 2024, which reflected R1 ran to R3 and grabbed R3 by the hair and staff intervened. 4. In an interview, E1 reported during R1 and R3 sit near each other during evening medication pass and R3 sits at dining table waiting for dinner, E1 reported R1 is often redirected and the staff tries to separate R1 from R3, however R1 refuses the redirection.
