A licensure complaint, prompted by #CO38983, was completed on 1/22/25. A deficiency was cited. Based on interview and record review, the residence failed to require staff members to document, before the end of their shift, any out of the ordinary event or issue regarding a resident that they personally observed or was reported to them, affecting one of three sample residents (#1).Findings include:Resident #1 was admitted to the residence on 5/24/2023 with a diagnosis of Alzheimer' s disease.A practitioner' s note, dated 1/14/25, read in part that Resident #1 had a rapid decline and was transported to the hospital.However, Resident #1' s record did not include a progress note for his hospitalization on 1/14/25.On 1/22/25 at approximately 11:00 a.m., the wellness nurse (WN) said Resident #1 had a rapid decline and was not doing well, and he was transported to the hospital. She said there was no documentation of what happened before the residence sent Resident #1 to the hospital. The WN said the qualified medication administration person should have documented the event before the end of their shift. She said she also documented events in incident reports after a resident was hospitalized or had a change in their baseline status; however, she did not do so on 1/14/25.

A relicensure survey, with complaints #CO31639, #CO32997, #CO33605, #CO34113, #CO35339, #CO35406, #CO36571, #CO36668 was completed on 7/18/24. No deficiencies were cited. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.21 The assisted living residence shall be responsible for complying with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers. 14.29 All prescribed and PRN medications shall be listed and recorded on a medication administration record (MAR) which contains the name and date of birth of the resident, the resident' s room location, any known allergies, and the name and telephone number of the resident' s authorized practitioner. (C) Each qualified medication administration person, nurse, or practitioner shall accurately document each medication administration or monitoring event at the time the event is completed for each resident.
