FACILITY EVALUATION REPORT

The facility underwent a Required-1 Year Inspection conducted by LPAs Michael Hood and Marisa Chiarelli. The inspection covered general areas, including bedrooms, bathrooms, kitchen, and safety equipment. No deficiencies were cited per California Code of Regulations, Title 22.

The facility underwent an unannounced annual inspection conducted by LPAs. The inspection covered resident rooms, common areas, kitchen supplies, and medication storage. No deficiencies were cited during this visit.

This was a follow-up inspection conducted on 12/14/2023 to review the plan of corrections. While the LPA cleared deficiencies during the visit, several maintenance issues were observed, including floor holes, damaged kitchen shelves, inoperable locks, and missing ceiling smoke alarms.

The inspection identified multiple deficiencies, including a critical issue with hot water temperature exceeding safe limits (Type A). Additionally, there were multiple Type B deficiencies related to general facility disrepair, such as floor holes, broken locks, and improperly mounted smoke alarms. Immediate corrective action is required for the hot water temperature, alongside repairs for structural and safety equipment.

This report details a complaint investigation conducted on behalf of the facility. The investigation found no preponderance of evidence to prove the alleged violations, resulting in the allegation being deemed UNSUBSTANTIATED. Therefore, no deficiencies are being cited during this visit.
