Key Review Excerpts

The admin and clinical staff are attentive and communicative with patients and families. The facility is well kept inside and out. The residents are engaged with each other and staff.

Long-term resident's family · 2024★★★★★

Instead of watching and making sure she's OK I find her laying in her own Feces. You tell the staff you need help and they say sure I'll be there in a minute..yet they continue to stay seated watching shows or videos on their cell phones.

Rehab patient's family · 2023☆☆☆☆

I met the Social Worker Carlon Ruffin, who is very compassionate about the residents she serves. Ms. Ruffin was very helpful and took out the time to assist me when my husband expired.

This facility shows concerning patterns with families repeatedly filing complaints that triggered 20 of 41 deficiencies across multiple surveys. The most recurring problems involve basic daily care assistance, medication management, and infection control, with issues persisting across recent inspections rather than being fully resolved. Most deficiencies are marked for correction but given the pattern of repeat violations in core care areas and ongoing family concerns, visitors should carefully assess current conditions.
